Yeah, that's how it works, basically.
Most actions will only have a (1/20) chance to trigger Red Thread after a critical success (giving a total success rate of about 1/200, I think), but fishing doesn't have critical successes, so it has a natural 1/20 chance, and with a cost of twenty minutes as opposed to most actions being 10 minutes, this makes fishing about 5 times as efficient as any other action (barring kissing, which is five minutes, but I don't think it can actually trigger Red Thread). Fishing also doesn't check for a date, so you can get it anywhere, even with complete strangers.
