Well it depends, if Miko was a Guy during his life before Gensokyo, then Miko would be a unwilling MtF who still sees himself as a man (considering his group still calls him crown prince). Or if he was a girl before Gensokyo, then he's (I stoped caring about pronouns at this point) just a tomboy and the crown prince title is just something he feels comfortable with or is something passed down. Based off from reading the wiki, I think it's the latter since it wouldn't make sense that a preserved body would for some reason change its sex since its preserved and all, but this is Gensokyo (and ZUN) we're talking about so who knows.

Miko is considered a incarnation of Prince Shotoku, which would mean "reborn" and essentially lives through Miko in spirit.
It follows the concept of reincarnation in Taoism. Where in when one dies, they are reborn in to another being based on their previous lives.
