If Prince Shotaku can become an adorable girl, there's really no reason why not. Adorable little girl characters are basically a staple of Touhou - changing that would be like making the next game about modern-day Manhattan being attacked by aliens.

ZUN had his chance when he started talking about characters like Rinnosuke, Youki, Marisa's Unnamed Father... But he let the opportunity slip by and now there's just no way Touhou can be Touhou without its all-ALG cast.

Old men appearing on 2hu, the fandom did not freak out (Unzan on the game, Youki is still ambiguous, Myouren was scrapped later).

Rinnosuke somewhat serves as an adult figure in the official writing works, so the fandom is torn between hatred or being apathetic on him.

But the appearance of a teenage boy will enough to cause a freak out in the fandom.

Technically is not a boy. But rather an old man named Myouren Hijiri (sibling of Byakuren). ZUN felt weird to put an old man as a Final Boss because it's unusual, so he decided scrap it.

Or maybe he managed to put "Old Man" role as a certain pink cloud that followed a Buddhist nun.

I know, I know. What I meant is that imouto-chan Momiji was the popular interpretation that the fans had back then. ZUN, instead of pandering to this, basically said "how about no" and kept the character as she was originally envisioned. (Hopefully I made myself clear this time)

As for the "men" thing, I still find it strange of him (considering how he wanted to add Myouren at all), but whatever. As long as the games are fun and the music's good.

It's more the case that he had the idea of "Myouren" from the beginning, since his characterization is based on an actual priest, and that what was he wanted to add so he turned that idea into a sister instead. It isn't so much that he had a solidified male character that he was prepared to add but didn't. This was just his way of fitting an idea into a Touhou character at the time, whereas with the next game he chose a different method. Miko being based on a "historically" male figure but "hey female all along" and having the commentary on the debate of Shotoku's existence, is somewhat of the same thing.
