When do you use お/ご as a prefix? I understand it's supposed to be used when you want to be polite but it seems a little excessive to throw it everywhere and sometimes it is used awkwardly (like saying おまんこ during rape).

When? Well, there isn't really a rule for that, you can use it depending on how you want your sentence to sound, the same as a sentence with "desu" sounds "softer" than one without it.

Just listen to how it's used and get a feel for it yourself. In most cases, it won't really make any difference if you don't use it, but for other words the prefix is used so often that it's basically considered a part of the word, like, お腹,　お尻 and　お茶.

The only context I can think of where you'd want to attach it even to a word where you usually don't attach it, is if you request something from someone while being polite, like requesting a name by saying "お名前は？".
